It creates a compound failure: vulnerabilities are discovered at the most expensive point in the development cycle, when they require the most rework to address. The security team becomes a single point of failure and a delivery bottleneck. Developers experience security as adversarial friction that blocks their work rather than as a shared responsibility that protects it. And because the final gate is a pressure point, there is a natural organizational tendency to rush through it or find ways around it when deadlines approach.
The shift-left principle exists precisely to counter this design. When security controls operate earlier—in the IDE, in pre-commit hooks, in CI on every push—findings surface when they are cheapest to fix and when the developer who introduced them is still working in the relevant context. The psychology of ownership also shifts: a developer who sees a vulnerability flagged in their own PR is much more likely to engage with it than one who receives a bug report from an external team weeks later.
Anti-Pattern 2: Alert Fatigue from Untuned Scanners
Deploying security tools without calibrating them to the specific codebase and risk profile generates large volumes of low-confidence, low-severity findings. When developers are routinely overwhelmed with noise—hundreds of scanner outputs per build, most of which turn out to be invalid or acceptable risks—they begin to dismiss all scanner output. This is rational adaptive behavior in the face of signal that has been devalued by excessive noise. The consequence is that genuinely critical findings get buried with the rest. A single critical SQL injection finding on page 47 of a 200-item scanner report will not receive the attention it deserves.
The remedy is to start small: enable a minimal set of high-confidence, high-severity rules, measure the false positive rate, and expand the ruleset incrementally as the team builds confidence in the tool. Security scanners that developers trust get acted on; scanners that developers distrust get circumvented or disabled.
Anti-Pattern 3: The Siloed Security Team
When security teams operate as a separate organizational unit that interacts with development teams primarily through audits, security reviews, and vulnerability reports, the relationship is structurally adversarial even when all parties have good intentions. Security engineers don’t build a concrete understanding of the delivery constraints and technical context that developers work within. Developers don’t develop intuition for secure coding because they receive findings from a black box rather than guidance from a trusted colleague. Security policies are written without input from the people who must implement them, leading to requirements that are either ignored or implemented in ways that satisfy the letter of the policy rather than its intent.
Breaking down the silo requires structural changes: embedding security engineers in product squads for defined periods, running joint design sessions between security and development, and measuring collaboration rather than just compliance.
Anti-Pattern 4: Hardcoded Credentials and Secrets
Hardcoding API keys, database connection strings, OAuth tokens, and private keys directly into source code is one of the most preventable and yet most persistently common security mistakes. It typically happens under time pressure during prototyping: a developer embeds a credential to get something working quickly and intends to externalize it before the code is committed. This intention doesn’t always survive contact with a deadline. GitGuardian’s annual research consistently finds millions of valid secrets leaked to public repositories every year, with a significant number coming from accidental commits of keys that were supposed to be temporary. Pre-commit secrets scanning using tools like Gitleaks, TruffleHog, or GitHub’s native secret scanning capability should be a non-negotiable baseline in every engineering organization.
Anti-Pattern 5: Dependency Neglect
Modern applications commonly have hundreds or thousands of transitive dependencies. A pervasive anti-pattern is to add a library during initial development and never revisit it afterward. Over months and years, security vulnerabilities are discovered and disclosed in these packages. Without automated monitoring, organizations may be running vulnerable components for months without awareness. The December 2021 Log4Shell vulnerability illustrated this pattern at catastrophic scale: Apache Log4j had been quietly embedded in virtually every Java-based enterprise application and countless commercial products, and the effort required to identify, assess, and patch every instance while attackers were actively exploiting the vulnerability was enormous. Automated SCA tools with continuous monitoring and proactive alerting on new CVEs are the direct antidote to dependency neglect.
Anti-Pattern 6: Ignoring Infrastructure Misconfiguration
Application-layer security receives substantial attention, but a significant proportion of real-world data breaches originate from cloud infrastructure misconfigurations: storage buckets left publicly accessible, IAM roles with excessive permissions, databases exposed to the public internet, encryption at rest disabled to reduce complexity. These are not application vulnerabilities that SAST or DAST tools will detect—they live in Terraform files, CloudFormation templates, and cloud console configurations. IaC scanners and cloud security posture management (CSPM) tools address exactly this gap and must be treated as a core part of any comprehensive security program, not an optional enhancement.
Anti-Pattern 7: The “Ship It and Patch It” Culture
Under sustained delivery pressure, some teams effectively adopt an unspoken policy of shipping features with known security issues and promising to address them later. The problem is not the trade-off itself—there are legitimate situations where accepting short-term risk in exchange for business value is the right decision—but the absence of explicit acknowledgment and governance. When these decisions happen implicitly, through the organic prioritization of feature work over security backlog items, the security debt compounds without any formal record. Establishing a lightweight risk acceptance process—which requires explicit sign-off and a committed remediation timeline for any known vulnerability being shipped—forces these trade-offs into the open where they can be managed deliberately.

It does not deliver visible, user-facing value in a single sprint, it often requires expertise that is unevenly distributed across the team, and its business value is expressed in terms of risks avoided rather than outcomes delivered. In organizations where sprint velocity and feature delivery are the primary measures of team performance, security work is systematically underinvested—not because people don’t care about security, but because the incentive structure doesn’t reward it.
The opportunity is that short iteration cycles create frequent, regular checkpoints at which security concerns can be surfaced and addressed before they accumulate into expensive technical debt. A team that spends fifteen minutes at the start of every sprint review discussing the security implications of the features being planned will build security awareness organically, without the overhead of formal security reviews for every change.
Security User Stories and Abuse Cases
Traditional user stories describe the system from the perspective of a legitimate user trying to accomplish a goal. Security-oriented user stories—sometimes called “evil user stories” or “abuse cases”—describe the system from the perspective of an attacker trying to exploit it. Writing abuse cases alongside regular user stories helps developers think about adversarial scenarios during the design and implementation phases, where the cost of addressing them is lowest. Examples:
- “As a malicious user, I want to manipulate the JWT token payload so that I can impersonate another user’s session and access their private data.”
- “As an attacker, I want to submit crafted input to the file upload endpoint so that I can store a web shell in the application directory.”
- “As an unauthorized user, I want to enumerate numeric order IDs in the API URL so that I can access order details belonging to other customers.”
Abuse cases don’t need to be accepted into the sprint backlog to have value—even as a planning artifact that is refined and discussed, they generate security-relevant thinking at exactly the right time.
Security Spikes for High-Risk Features
When a team is about to implement a feature with significant security implications—a new authentication flow, a payment processing integration, a feature that handles personally identifiable information or protected health data, a new API that will be exposed to external consumers—it is worth budgeting a security spike at the start of the effort. A spike is a time-boxed research activity, typically ranging from half a day to two days, to investigate the security requirements, identify the correct implementation approach, evaluate available libraries and frameworks, and surface any relevant compliance obligations. The investment in a spike is almost always recovered by avoiding fundamental design decisions that would need to be reversed during implementation or code review. Redesigning an authentication flow after it has been integrated across six services is orders of magnitude more expensive than getting the design right before coding starts.
Allocate Dedicated Capacity for Security Work
Development teams should maintain a dedicated security backlog alongside the product backlog and protect a fixed percentage of sprint capacity for addressing it. This backlog contains items like dependency updates addressing known CVEs, implementation of new protective controls, resolution of technical debt in existing security-sensitive code, and completion of security testing for features that were shipped with deferred testing. Industry guidance for teams with moderate security debt typically suggests allocating 10 to 15 percent of sprint capacity to security and technical debt work. The specific allocation matters less than the consistency: a team that reliably reserves even 10 percent of capacity for security work will make steady, measurable progress. A team that treats security work as overflow—addressed only when no feature work is available—will find that security debt grows indefinitely.
Security in the Definition of Done
The Definition of Done is the shared checklist of criteria that every user story must satisfy to be considered complete. Teams that effectively operationalize security include security criteria in their DoD as non-negotiable standards. Examples might include: all new API endpoints validate and sanitize input; no new high or critical severity vulnerabilities are introduced, as verified by the CI pipeline security scan; sensitive data fields are identified and appropriate encryption or access control is applied; authentication and authorization requirements for the new feature have been explicitly tested with both positive and negative test cases. By encoding these requirements into the DoD, the team creates a structural expectation that security is a property of completed work, not an optional attribute.
Secure Code Review Practices That Don’t Kill Velocity
Code review is one of the highest-leverage security controls available to a development team. Experienced reviewers catch logical vulnerabilities, broken access control patterns, business logic flaws, and architectural concerns that no automated tool is capable of detecting. At the same time, poorly structured code review is a reliable way to create delivery bottlenecks, breed resentment between reviewers and authors, and generate superficial approvals driven by time pressure rather than genuine scrutiny. The challenge is to maximize the security signal from review while keeping cycle times short and reviewer attention focused on the issues that matter most.
Let Automation Handle Pattern-Based Issues
Automated security tools should have already analyzed every pull request and posted their findings as inline review comments before any human reviewer opens the diff. This division of labor allows human reviewers to direct their attention entirely to issues that machines cannot detect: business logic vulnerabilities, incorrect threat model assumptions, access control decisions that violate the principle of least privilege, race conditions in concurrent code, and the subtle interaction effects between new code and existing security controls. Reviewers who spend their time commenting on issues that a linter should have caught are reviewers whose time and attention is being wasted.
Use Context-Appropriate Security Checklists
Rather than expecting reviewers to reconstruct every applicable security consideration from memory for every review, provide concise, context-specific security checklists as pull request templates. A template for authentication changes might ask: Is the session management implementation consistent with the team’s approved pattern?
